Community Living Corporation is a nonprofit organization that provides residential and day services to developmentally disabled adults in Northern Westchester County. Community Living Corporation is seeking a Licensed Practical Nurse (LPN) to join our team!


Job Summary:

The Licensed Practical Nurse (LPN) is to provide high-quality care to all the clients residing within Community Living Corporation. The LPN will play a crucial role in delivering compassionate healthcare, promoting patient well-being and supporting the overall care team in a residential or day program setting. The ideal candidate will have strong clinical skills, knowledge of OPWDD regulations and a commitment to patient-centered care.

Community Living Corporation is seeking a full-time LPN (Licensed Practical Nurse) to join our organization. Duties include but not limited to:

Essential Functions:

  • Provide nursing care, including medication administration, monitoring vital signs and basic wound care.
  • Responsible for the execution of medical regimens prescribed by a Licensed Health Care Professional authorized to order such care daily.
  • Develop and implement care plans in collaboration with Registered Nurses (RNs) and health care providers.
  • Monitor and document the health status of individuals with developmental disabilities in accordance with OPWDD regulations
  • Support the physical, emotional and social needs of individuals in a residential or day program environment
  • Communicate effectively with the clients, families and interdisciplinary teams to ensure continuity of care.
  • Maintain accurate and detailed medical records in completed with OPWDD standards
  • Assist in training and education Direct Support Professionals (DSPs) on health care protocols and procedures
  • Respond to medical emergencies and follow established procedures for managing patient care
  • Ensure compliance with all health care and safety regulations, including OPWDD policies and procedures

Prior experience working with developmentally disabled individuals highly preferred. Ability to interact and communicate well with individuals with developmental and psychiatric disabilities.

Must be a Licensed Practical Nurse in New York State. Must have valid driver's license for a minimum of 3 years, and own transportation. Background check and drug testing required.
